Frankly I was not expecting Thatcher's dementia as a framing device but it made telling the story more interesting, by turns tragic and funny (my favourite scene is probably the one where she admonishes the MD) The film may play better w/those who are cognizant of events during her tenure because it's not a straightforward biopic so you need to know a little recent UK history. Of course Streep is spooky good and the ever-dependable Jim Broadbent provides excellent comic relief; both are Oscar worthy. One may not entirely admire or like Thatcher but she had more guts than most of the men around her, that's for sure.
